The visual confirmation image has alpha characters in caps, and newbies typically have their SHOUTLOCK on anyway...

Bleh, people should be happy with capital letters in these images. My CVS doesn't even have working images

If the image was: ASDF and you typed in: asdf it would be wrong.

I don't see a problem with that, capital letters ARE different from normaly letters, just as long as they are nice and clear so users can read them easier, and there is the option to refresh if the image is difficult to read then it should okay.
And probably have a message like:
Please enter the security code in the image above. Note that this is case sensitive meaning it treats upper and lower case letters as seperate. THESE ARE CAPITAL LETTERS!!!!! these are not

you have to take into account some "older people" use the boards, oap ect and they do need it spelled out at them as far instructions go

The letter O appeared in my code. Couldn't tell if it was an O or number 0.
Is there a way to set the size of that image? I do have people who are pretty much blind, and some that uses Lynx. How will they register?

You can always turn it off. Or if you want to keep it and there's a minor few that can't use it then you can setup an account for them. One forum that I use does have a handful of users who have very poor eyesight. Using the forum doesn't present too much of a problem since they can just increase the text size, but obviously it is more difficult to resize images.

Captchas
Is there going to be put more effort/obscuring in the images in future releases? I am not saying that is absolutely necessary, I am just wondering.
The current c't issue (german computer magazin) has an interesting article on Captchas (Completely Automated Public Turing test to tell Computers from Humans Apart), which basically talks about such images which are easy to read for humans but almost impossible (if done right) for computers.
For example Yahoo uses EZ-Gimpy[1] which is sad to be algorithmically solved by Malik and Gori from Berkeley[2] (though it is not publicly available and probably won't be for some time), as are other captchas.
The article also talks about methods of 'attacking' captchas, which goes from brute-force dictionary, over erosion/dilatation and to Shape Context Methods[2].
Like I said, I don't think it is crucial for the board-software, however I thought I'd throw it in, in case somebody's interested in it, or thinks of [easy] ways to improve the phpbb-captchas, by rotating and/or distorting the letters (No, I didn't check what the img-manipulation capabilities of php are.

Further googling for captchas[3] finds interesting further readings. /me wanders off reading more about captchas on the web...
