Is it possible to modify this MOD to specify that a specific ad will appear only in a selected forum and not all forums?

Also, does a code exist to place ads on the Index page "in between" the various forums, above the "Category" headers ?

1. It's likely possible but Exreaction would be the one to ask for such a modification to his modification. Try his site. That's not exactly support so I don't think a fee would be involved but don't quote me on that.
2. It would be pretty hard to do that. You could just create a forum of type link, show your image and description if applicable and have it redirect to wherever it is you want the ad to go.

1.Yes it could be done you have to create Separate Ads and set the forums in which you want to display them.

1. Yes, but I would recommend you check out the Forum Sponsors mod, which was designed to specifically do that.
