Inline it's done as
<link rel="SHORTCUT ICON" href="URL to favicon.ico">

can't find where it's done within an external style sheet at the moment.

the .ico only works on windows... I believe that the Standards-complaint is <link rel="icon" href="URL to favicon.png"> (with PNG being the most common used because of it's Alpha transparency)...

If you specify both, IE takes the .ico, and Firefox uses the ICO, unless you have specified a different one with the rel="icon"
