Check your settings in ACP>>General>>Board Configuration>>Visual Confirmation Settings
If you have changed those simply hit reset than see if they work.

Well i just saw that version is outdated.Use this instead it requires no edits at all.

Images/avatars/upload/:777
and its not 750 its 755 for all other directories.
