The Template Refresh Buttons - They Do Nothing!

Hello I've been editing my templates, however, try as I might I cannot get them to refresh. I have tryed deleteing my temporary files and cookies in my browser and using the ACP to refresh everything : no luck at all.

Which brings me to the real question and perhapse the questions that tag along with it:

What is the proper way to create a new custom page like this? Why is PhpBB set up like this ( for speed )? After making a call to a database, do we need to
// free result set memory
mysql_free_result ...
