Could you now show me a .txt version of admin/board_config_body.tpl, so I can have a look at that.
In this situation, I would suggest that you run lev_mod_db_setup.php from the phpBB 2.0.x root directory, but you say you already tried that.

The reason why they need to be in a sub-folder, is because EasyMOD only searches through sub-folders for an install file.

OK, I have installed this MOD on a clean phpBB 2.0.22 installation, and it works fine for me.
The radio options in /admin/admin_board.php should be at a default of "yes".
I'll have a look at this later on today; I'm going to the doctors now.
