Yea, from what I remember, after you create a new forum it says "You can now set the permissions for this forum".

Maybe something along the lines of "You must now set the permissions for this forum before you can use it" would be suitable?

I've had this happen with custom bbcodes before, I'm pretty sure its a caching issue.

If you clear your cache I bet it will work.

I'm guessing it must be some pretty rare/unstable modifications then if there are no obvious examples.
