That's a very tricky question. You will of course need FTP access to get the actual modifications onto the server, but assuming something like EasyMOD is built in, the actual installation will be done for you.
If you are asking because you are on a free host, they will likely disable any feature ...

It may be possible to use something like SMF's package manager (which isn't all that special BTW), but it is not possible to do a Firefox like update because phpBB runs on a server and not your local machine. My guess is close to what Lastof posted above. It is impossible to provide upgrade paths for all MODs because there are simply too many. In fact, there are at least 3 birthday MODs that I am aware of.
