Hello Callum,
you can handle rare false positves (I don't have them on my boards.) with redirecting them to a contact form. The contact form can be protected e.g. with CAPTCHA.

The problem with HTTP blacklists is that every HTTP blacklist uses an own different API. There is no standard like it is for (most of) DNS blacklists.

Your MOD/code is a very carefully solution. Which means that moderators have to check every post which has been called to be spam by Akismet. (I hope that I'm right.) I understand this point. But I could imagine that moderators won't like ...

absolutely +1! It's just state-of-the-art.

Make a combination of:
Add a field/column called style_type to STYLES_TABLE. Use this column in combination with:
Add auto detection for mobile devices (non JS).
Add a new user_setting and board_setting for the mobile style.

I would be nice if a phpBB AJAX documentation for MOD authors would be available then the first beta of Ascraeus is released. So we (MOD athors) will have a chance to use it.
