Did you purge?
ACP -> General tab -> Purge the Cache button

Did you refresh cache on your styles when you upgraded?
ACP -> Styles tab -> Style Components -> Templates -> Refresh link

Via FTP delete all files in your cache folder with the exception of .htaccess and index.html

'FORUM_DESC' => generate_text_for_display($row['forum_desc'], $row['forum_desc_uid'], $row['forum_desc_bitfield'], $row['forum_desc_options']),

In the SRT you answered the following: "Did you use an automated wizard provided by your host to install phpBB? yes"
If you installed using your hosting control panel the update may fail due to file differences. Installs of this type are not supported for that reason.
You may want to overlay all files except your config.php file with a fresh full version of phpBB 3.0.4 then run the database update by running from your browser http://yoursite/phpBBfolder/install/database_update.php. You will lose all MOD's using this method.

Yeah, I never use cPanel or Fantastico versions. I ran into trouble, very early on, and learned my lesson about that. Always best to get the 'fresh' copy of any software packages. Nice detective work PitBull!
