This appears to be a common problem on Bluehost. Do NOT use Pagebreeze to access your forum files. Use an FTP program to access your web server. If you don't have one, then read: Knowledge Base - Tools needed to set up and customise phpBB If you don't know your FTP details, then the web host company should be able to tell you. Please DON'T go trying random things, you could destroy good data with bad.
You host must provide some sort of toolbox for controlling your website. Look in there to see if there is an option to view log files.

Where are you trying to install this? i.e. on what web host?
There seems to be something unusual about their PHP installation, which is confusing the installer when it checks what databases are available.

Did you have any MODs (modifications) installed?
If not, then you can do a clean update to 3.0.4, replacing all your files as described here: docs/INSTALL.html#update_full

Also, even if you did have MODs, you can do the clean update, and then re-install the MODs.

forums.domain.com and forums2.domain.com are NOT the same domain, so as Brf noted, you can't do this.
For the convertor to work, the two boards must be on the SAME domain.
Just install phpBB3 into a folder in forums.domain.com. It is easy to move it to the root when you are finished.
