It may be possible to use something like SMF's package manager (which isn't all that special BTW), but it is not possible to do a Firefox like update because phpBB runs on a server and not your local machine. My guess is close to what Lastof posted above. It is impossible to provide upgrade paths for all MODs because there are simply too many. In fact, there are at least 3 birthday MODs that I am aware of. There should be an update path provided for the attachment MOD (as stated in the in the FAQ topic),...
