It would be good if phpBB didn't allow you to set the upload limit any higher than the PHP limit, actually. Must remember to check if this has ever been reported in the bug tracker (I don't think it's a good thing the software is just sitting there, with not indication that something is wrong other ...

Don't worry too much about the "file not found" entries. Look for PHP errors (if you search for the string "PHP" you should be able to find them).

This sounds like the phpBB error log. I am talking about the web server's error log. I.e. the error log generated by Apache or whatever other web server you use. You may have an option to view that from the control panel provided to you by your web host. Otherwise, you will need yor web host's help ...

From what version did you upgrade? These settings have changed recently, from being a set of radio buttons to being a set of checkmarks. Are you sure you executed the upgrade correctly? Changed the files and ran the database upgrade script?
