Perhaps I have missed something but if subfolders for attachments has been added to 3.3 how do you use it?

Let's play a bit of Devil's Advocate here. What advantages will Doctrine give to phpBB other than not having to maintain it? Are there features/facilities in Doctrine that phpBB needs ? Are there currently any issues with phpBB's DBAL? How much time/effort is spent maintaining phpBB's DBAL? How much...

Is this phpBB related as neither of those files exist in phpBB

I suspect those settings are from one or more extensions

Don't know what that means. Each extension can be enabled/disabled individually now - in fact that is the way it works.
