1. Your probably using a old CVS version. The news one should work fine.
2. For Unix/Linux servers, as Trek said, you should chmod it to 777. I however, didnt do this, and it still created directories fine =.
3. If all else fails, make these folders: cache>templates>subsilver

Fix: Copy the table named [forum sql prefix].phpbb_config and rename it to phpbb.phpbb_config.

No, there will not be a update (automatic that is) to 2.2. You can however, read the changes and apply them yourself.

Hrmm, methinks if Karma were represented by numbers (e.g. +10/-12), it would be better? I mean, we all know psoTFX has super high +/good karma, but how about knowing how high he really is? Make people strive to have as little -'s as possible.

Keep in mind this is a Alpha. Also, if you dont like it, it is easy enough to change on your own. Now, if your talking "out of the box with it on the left", I dont think they would change it.
